Breathwork Waiver

By participating in any breathwork session, class, or workshop offered by Crystal, you understand and agree to the following:

1. I understand that breathwork involves intentional breathing techniques that may produce emotional, physical, and energetic responses. These can include tingling, light headedness, temperature changes, emotional release, or physical sensations.

2. I acknowledge that breathwork is not a substitute for medical care and should not replace advice or treatment from my healthcare provider.

3. I confirm that I am participating voluntarily and take full responsibility for my physical, emotional, and mental well-being during and after the session.

4. I understand that it is my responsibility to notify the practitioner of any health conditions, concerns, or medications that may affect my participation.

5. I release Crystal and all associated facilitators from any liability for harm, injury, or complications that may arise from my participation.

6. I understand that I may stop the session at any time if I feel discomfort or need a break.

7. I confirm that I am at least 18 years of age (or have guardian consent).

Colon Hydrotherapy Disclaimer

I understand that Colon Hydrotherapy, also known as colonic irrigation, is a natural therapy intended to promote digestive health and detoxification. This procedure is not intended to diagnose, treat, cure, or prevent any disease. I acknowledge that Colon Hydrotherapy is not a substitute for medical care, and I should consult a licensed medical professional for any health concerns.

I understand that Colon Hydrotherapy involves the introduction of filtered temperature-controlled water into the colon to help remove waste. While generally safe, possible risks include dehydration, electrolyte imbalances, discomfort, cramping, or temporary changes in bowel habits. I acknowledge that I am responsible for informing my practitioner of any concerns or discomfort during the session.

I confirm that I have disclosed all relevant health conditions and medications that may affect my ability to receive Colon Hydrotherapy safely. I understand that I should not undergo Colon Hydrotherapy if I have any of the following conditions:

• Active gastrointestinal infection

• Severe hemorrhoids

• Uncontrolled high blood pressure

• Recent abdominal or rectal surgery

• Pregnancy (especially in the first and third trimesters)

• Cancer of the digestive tract

• Severe anemia

• Kidney or liver disease

• Any other condition as advised by a medical professional.​

Home Liability Waiver

I acknowledge that participation in any services with the Dharma Den is covered under this waiver. It is valid for my participation in any event or service regardless of the date of same and shall extend to future events. and assume all risks associated with yoga, all body work, breathe work, women’s circles, workshops, cold emerge, sauna at 21034 High way 48 Mount Albert , and hereby release and forever discharge, the Addressees, from any and all liability associated with the Event, including without limitation, all falls, effects of cold immersion and/ heat, and general conditions  I confirm that I have addressed medical concerns, if any, with my doctor, and if applicable, I have been cleared to participate.  I am participating willingly and without coercion.

 

I have read and fully understand this waiver and in consideration of the acceptance of my entry for myself and anyone acting on my behalf to the Event.  I waive and release, Crystal Veenstra-Jarvis and/or The Dharma Den and its executing employees, volunteers, agents, successors and assigns, including but not limited to A&J Farms.inc from any and all claims, liabilities or causes of action, including, without limitation, death, bodily injury, property damage, or any other loss, damage or any inconvenience whatsoever arising from my participation in this event.

 

 

Photo & Video Release

I grant full permission to Crystal Veenstra- Jarvis to use photographs, videos, and other types of recordings of me in advertising, trade or other commercial purposes in legitimate accounts and promotion of this event.  I waive the right to inspect versions of my image used for publication or the written copy using in connection with the images. I also grant use of my email address for mailing lists.  I can unsubscribe at any time.
